Exit Criteria
It is expected that once a child is referred to Great Steps Ahead he or she will remain in the program until developmental level is age appropriate or when the child has reached the age of 3 years. At that point, children will be referred to other services based on need and parent priorities.
Children served by GSA may be exited from the program in the following of ways:
1. Family requests discontinuation of services in writing or by calling the Kern Regional Center Service Coordinator.
2. Family is terminated due to non-attendance with due process.
3. Home situation/condition or environment is determined to be a danger to the specialist or service provider. Such circumstances might include but are not limited to: domestic violence, sexual advances, uncontrolled animals, verbal and physical aggression and drug traffic. Documentation will be made in writing of any such incidents.
4. The child is determined to be no longer in need of services and the IFSP reflects developmental levels within normal limits as evaluated by formal assessment tools.
5. The child has reached a chronological age of 3 years.
Client Records
The Director is responsible for the maintenance of all records. Strictly confidential individual client case files are maintained in the office of Great Steps Ahead which contain the following documents:
- Referral Sheet
- Contact Sheet
- Assessment authorization
- Authorization to Obtain or Release Information
- Initial Home Interview or Visit
- Weekly Session Reports
- All program developmental assessments and semiannual reviews
- All requested medical information
- IFSP
- Referring agency medical, social, developmental and, psychological reports
- Special incident reports
- Emergency Information
- Authorization for emergency medical treatment
Confidential information may only be obtained or released by Great Steps Ahead through written parent authorization or court appointed entities. A copy of that authorization is maintained in the office file.
